2025 Diversity Champion Award Nomination Form

The IDSA Foundation Diversity Champion Award honors IDSA members who exemplify extraordinary commitment to promote Inclusion, Diversity Access, & Equity (IDA&E) within the ID community.

The deadline for submission is October 6, 2025.

The 2025 Diversity Champion will be announced at the Culture Club: A Celebration of Diversity held at Ventanas, on October 21, 2025, and featured in IDSA Foundation communications.

Eligibility

Self-nominations and peer nominations are welcome.

All nominees and nominators must be a current IDSA member in good standing.

Nominations are open to physicians, researchers, educators, public health leaders, and other infectious diseases professionals.

Board members are not eligible to be nominated, but they are welcome to submit nominations.

Nomination Criteria

Nominators must submit a 750-word statement highlighting the nominee’s accomplishments and contributions.

Provide examples of impact in advancing health equity, community engagement, workforce impact, and/or advocacy and education (mentorship).
